Registration of new patients with the practice
To register with the practice to receive all services from Moorside Surgery, including home visits and other out-of-hours services we will need to see proof that you live within the practice boundary. This is normally in the form of a utility bill, bank statement or similar document dated within the last 3 months and showing your current address. You will then be asked to make an appointment with the practice healthcare assistant for your new patient health check.
Find your NHS number
You do not need to know your NHS number to use NHS services, but it can be useful to have it.
